Client Services Agreement & Informed Consent
Nature of service. Soul Compass provides non-clinical coaching, supportive guidance, education, structured reflection, parent support, workshops, and facilitated conversations. Services may help you explore concerns, identify goals, build practical skills, and make your own decisions. No particular result is promised.
Your responsibility. You remain responsible for your choices, actions, personal safety, and medical or mental-health care. You may decline a question, request a pause, or end services. We may recommend a licensed professional or discontinue services when your needs fall outside our scope.
Relationship. This is a coaching/consulting relationship. It does not create a therapist-patient, physician-patient, attorney-client, fiduciary, guardian, or emergency-response relationship.
Communication. Scheduling is handled directly by phone, text, or email. Sessions may be in person, by phone, or through Zoom, FaceTime, or Microsoft Teams. Messages may not be seen immediately.
No recording. No one may record, photograph, screenshot, livestream, transcribe with an automated tool, or use an AI note-taker during a private session, family session, group, or workshop without advance written permission from Soul Compass and every affected participant.
Ending services. Either party may end services. Soul Compass may pause or end services for nonpayment, repeated missed appointments, harassment, threats, boundary violations, conflicts, needs beyond scope, or safety concerns. When reasonable and safe, referral information may be offered.
Acknowledgment
By signing the final acknowledgment page, you confirm that you understand the non-clinical scope of services and voluntarily choose to participate.
